Regular Account Application

Section 1: Your Account Registration
Please provide the personal information requested in one of the choices below (do not fill out more than one part). If you choose Part A, include only one social security number (either owner or joint owner, but not both). If you choose Part C (Trust), please attach the first page and signature page of the Trust to the Rydex application.

Section 2: Your Address
Please provide your current mailing address. Account Confirmation and Statements as well as requested check redemptions will be sent to the address provided on the application, unless otherwise specified.

Section 3: Investment Selections
When choosing funds in Section 3, please remember that these choices are only your initial investment and that you can trade among all Rydex Funds at a later time. If you leave this section blank, your money will automatically be placed in the Rydex US Government Money Market Fund.

Section 4: Signature
Please sign the application. When you send the application to Rydex, you must send it with the original signature. A photocopied signature is not sufficient.

Section 5: Telephone Redemptions and Exchanges
This section of the application is optional. Redemptions will be sent only to the address on the application unless an alternative bank or brokerage account is on file at Rydex. Any changes to information provided on the account application must be made in writing with a signature guarantee. Withdrawals of $5,000 or more may be wire transferred to the bank of record. Wire redemptions of less than $5,000 will be charged a wiring fee.

Sections 6 & 7: Advisor
If you are investing directly, without appointing a Registered Investment Advisor as agent of your account, leave Sections 6 and 7 of the application blank.
